Levels of In-Home Personal Care and our Rates RATES AS OF MARCH 2O10 (SUBJECT TO CHANGE) CARE LEVEL l – recommended for care recipients who usually live independently at home but need some basic assistance in activities of daily living such as eating, bathing, toileting, walking, medication reminders, respite care, personal hygiene, companionship. Homecare duties such as light housekeeping, meal planning & preparation, shopping, transport to doctor’s appointments (small mileage fee per mile for driving may apply, pls. refer to rates below). 24-hour Live-in Shift 8-hour Live-out b 5-hour Live-out   $180.00 per day (24 hrs live-in) $17.00/hr d $18.00/hr CARE LEVEL 2 – MODERATE CARE Includes all duties listed in CARE LEVEL 1 plus moderate level of personal care that includes partial lifting of  wheelchair bound  recipients. Specialized care to patients with Dementia, Alzheimer’s, Parkinson’s Disease, Cancer, Multiple Sclerosis, chronic illness, those recovering from surgery. Use of specific adaptive equipment, bladder and bowel care. 24-hour Live-in Shift 8-hour Live-out rate 5-hour Live-out rate      $190.00 per 24 hr live-in shift d    $19.00/hr    $20.00/hr  CARE LEVEL 3 – PREMIUM CARE  includes all duties listed under CARE LEVEL I and 2 plus:  total bed care, non-sterile wound care, routine catheter care, follow a medically supervised plan of care, assistance with ambulation, exercise, transfers, proper positioning, hospice care, patients on GI-tube feeding, tracheal tubes, cleaning of ostomy bags, catheter and other assistive equipment or appliances. 24-hour Live-in Shift d 8-hour Live-out rate d 5-hour Live-out rate       If driving is required, mileage of $0.55 per mile applies to cover cost of gasoline & use of caregiver's vehicle.       $200.00 per 24-hr live in shift     $21.00/hr      $22.00/hr
